Job Description

We are seeking a compassionate and driven Nurse Health Coach to join our remote wellness strategy team. In this flexible role, you will empower individuals to achieve their health goals by providing personalized coaching, education, and support. You will work with a diverse population, guiding them through health challenges, chronic disease management, and lifestyle modifications. This position requires a proactive approach to member engagement, excellent communication skills, and a strong clinical background to deliver evidence-based health coaching interventions. This is a 100% remote position, offering the flexibility to work from your home office.

Key Responsibilities

  • Conduct comprehensive health assessments to identify member health risks and goals.
  • Develop personalized health action plans in collaboration with members, focusing on achievable and sustainable changes.
  • Provide one-on-one and group health coaching sessions via phone, video, or secure messaging.
  • Educate members on various health topics, including chronic disease management, nutrition, exercise, stress reduction, and medication adherence.
  • Monitor member progress, provide ongoing support, and adjust coaching strategies as needed.
  • Maintain accurate and confidential member records in compliance with HIPAA and company standards.
  • Collaborate with interdisciplinary teams, including physicians, dietitians, and care managers, to ensure holistic member care.
  • Utilize motivational interviewing and other behavior change techniques to foster member engagement and self-efficacy.
  • Participate in ongoing training and professional development to stay current with health coaching best practices.

Required Skills

  • Active Registered Nurse (RN) license in at least one US state (and ability to obtain licensure in other states as required).
  • Minimum of 2 years of clinical nursing experience.
  • Proven experience in health coaching, case management, or patient education.
  • Excellent verbal and written communication skills.
  • Proficiency in using electronic health records (EHR) and virtual communication platforms.
  • Strong empathetic listening and motivational interviewing skills.
  • Ability to work independently and manage a caseload effectively in a remote environment.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) degree.
  • Certified Health Coach (CHC) credential from a recognized organization (e.g., NBHWCC, ICHWC).
  • Experience with chronic condition management (e.g., diabetes, heart disease, obesity).
  • Bilingual proficiency (especially Spanish) is a plus.
  • Familiarity with digital health tools and wellness technology.
