[Remote] Business Project Manager-Healthcare (NO C2C)

The Business Project Manager in Healthcare will lead a multidisciplinary team to provide holistic care for a diverse health plan population, acting as the primary contact for members and caregivers.
Responsibilities
- Lead a multidisciplinary team—including social workers, dietitians, pharmacists, and clinical support staff—to deliver care telephonically and digitally
- Assess members’ health, psychosocial needs, cultural preferences, and support systems
- Develop and implement individualized care plans in partnership with members and caregivers, addressing gaps and barriers to improve outcomes
- Coordinate resources such as community services, mental health support, disease-specific programs, and transportation or durable medical equipment
- Provide education on health literacy, self-management, medications, and nutrition
- Advocate for members while promoting self-advocacy and engagement in care
- Monitor and adjust care plans as needed, documenting interactions and progress accurately
- Facilitate smooth transitions from facilities to home or other care settings, ensuring continuity of care
- Maintain compliance with professional standards and pursue ongoing development, including Case Management certification (CCM)
Skills
- Nursing Diploma or Associate's degree required
- 3+ years of clinical nursing experience in acute, post-acute, or community settings
- Current, unrestricted Michigan RN license required
- CCM certification required or to be obtained within 18 months
- Bachelor's degree strongly preferred
- Case management experience in a managed care setting preferred
- Experience with telephonic and digital patient management preferred
- CCP certification preferred
