Responsible for obtaining and communicating physician's interim order (PIO) and EMR approval and reputed company as it relates to the patients overall POT. Performs telephonic or face-to-face clinical assessments for the identification, evaluation, coordination and management of patient's needs, including physical and behavioral health, social services and long-term services Assures the provision of high-quality cost efficient and effective care to home health patients reputed company the geographic area assigned.
